Assertive Community Treatment

Assertive Community Treatment (ACT) is an evidence-based service designed to reduce psychiatric hospitalization and utilization of acute care, increase housing stability and improve quality of life for individuals living with severe mental health disorders and whose needs have not been met by more traditional mental health services.

ACT services focus on the unique needs of each individual, and service plans reflect individual strengths and life goals. The ACT clinical team consists of psychiatric medication providers, mental health treatment providers, nurses, substance use treatment providers, peer specialists and employment specialists who work together to provide comprehensive care with the goal of improving individual health, wellness and quality of life.

ACT is a voluntary program and based upon individual choice. Services are delivered in a variety of settings and for as long as they are needed.

ACT is an approach aimed at keeping people in contact with critical treatment and support services to improve health and quality of life.

  • 24-hour crisis counseling
  • Individual assessment and treatment planning
  • Nursing services, including care coordination, education, consultation and support
  • Treatment for co-occurring substance use disorders
  • Psychiatric services
  • Supportive Mental Health therapy
  • Skills-training to support and sustain stability
  • Vocational services, such as job interest assessment, education, follow-along supports
  • Case management to assist members in accessing benefits, housing and other support services
  • Education, support and consultation for families and other supports
  • Referrals and coordination of hospital admissions and discharges
